EVOLVE Partial Hospitalization Programs (PHP) support adolescents 12-17 years old who live at home but struggle to maintain balance due to mental health, addiction, and behavioral issues.
At the partial level of care, teens receive a full-day of treatment at Evolve, but continue to live at home. Partial hospitalization is an effective alternative to residential or inpatient treatment and may be appropriate for those transitioning from a residential treatment program or those who need more structure than a typical outpatient program or private psychotherapy.
The PHP level of care provides five hours of treatment, five days a week.
Experienced, Professional Staff
Our staff is handpicked to provide the highest level of care available. On-site staff includes nurses, residential counselors, and licensed therapists. Our counseling staff is made up of experienced clinicians, masters-level interns, and certified recovery coaches who bring a diverse and eclectic background to the treatment experience. While it’s not always possible, we do our best to have one staff member on-site who has kids of their own. We understand the experience of being a parent goes a long way when helping teens through the challenges of substance issues, behavioral problems, and emotional challenges.
Our teens receive solution-focused, data-driven, evidence-based, results-oriented therapy. Everything we do is geared toward changing behavior and finding the right combination of treatment and support. Since no two teenagers are alike, no two treatment plans are alike. During the admissions process, we conduct a thorough assessment, establish a baseline, and create an Individual Recovery Plan based on specific needs and goals. We know the rest of the world doesn’t pause for our teens or their families during treatment. In our Partial Hospitalization Program, teens continue their school work with the support of our education staff. We offer career and college counseling, vocational opportunities, educational testing, and student advocacy. Each center is equipped with a state-of-the-art computing facility that enables students to access public and/or private online learning programs and resources.
From the moment a family commits to our program, we begin work on a transition plan, with a focus on personal growth, achievement, and stable, sustainable integration into the most positive situation possible after treatment.
Evolve works closely with parents, schools, and referring providers to ensure a seamless continuity of care between levels of therapy. The Evolve team gathers critical data and engages in dialogue regarding problem and treatment history, strengths and deficits, and avenues for growth.
