Evolve is dedicated to working with our clients’ treatment providers.

Whether you’re a therapist in private practice or you represent another treatment center, our priority is coordination of care. From determining level of care to treatment and discharge planning, our team values communication. 

While parental involvement is of primary importance during the admissions process, independent treatment providers can also be of help in several ways. 

First, we want to understand your client’s symptoms and treatment history as completely as possible so that we can determine whether Evolve can offer the appropriate level of support. If you are referring your client to residential treatment, we may ask you to compose a letter of recommendation explaining why you believe your client requires 24/7 supervision at this specific time. This information not only helps our treatment team develop a full picture of the client’s needs, but also aids in the process of securing authorization for residential treatment from insurance providers. 

During the admissions process, we may ask parents to fill out a Release of Information (ROI). This enables us to receive an accurate treatment history of the client, collaborate on their treatment plan, and stay in touch with you, the provider, regarding their treatment progress. Upon discharge, we will collaborate with you and the family to ensure a smooth transition back into your care once the client has achieved a level of stability deemed safe for them to return to the home environment.
