Evolve Elk Grove Care Team

Your teen will work with a team that includes psychiatrists, therapists, and clinical staff who collaborate closely throughout treatment. Care is coordinated across the entire team to ensure consistency and alignment at every stage.

At our Elk Grove location, clinical leadership work closely with the entire team to provide individualized care throughout treatment. This includes:

“ At Evolve Elk Grove Outpatient, we are committed to creating a space where adolescents and their families feel safe, respected, and genuinely welcomed from the moment they walk through our doors. We understand that seeking support can be challenging, and our goal is to foster an environment built on trust, authenticity, and connection. It is important to our team that our clients and their families gain something from their time here; we teach coping skills, ways to improve communication, and provide psychoeducation about behavioral health in adolescence. We want to support our client and families as they navigate life’s challenges and discover their strengths.”

Tracking Progress & Outcomes

We use tools like the K-CAT to track each teen’s clinical progress throughout treatment, giving families measurable insight into how care is working over time.

Structured Programming

Each day follows a consistent schedule to help teens stay engaged and supported.

Family Support

Teens and parents take part in weekly family therapy focused on communication and practical, skills-based strategies families can use at home.

Crisis Planning

Therapists help teens and families make cope ahead plans and clear strategies for managing challenges outside program hours.

Discharge Planning

As teens near completion, therapists help families prepare for the transition and coordinate continuity of care with next providers.

Evolve Elk Grove Clinical Approach

Evolve’s therapeutic approach is evidence-based and rooted in behavioral care. Each treatment plan is tailored to the teen’s individual needs, using a combination of therapies that support long-term progress.

How We Approach Substance Use Treatment

For teens struggling with substance use or addiction, Evolve incorporates SMART Recovery, a science-based approach rooted in CBT. SMART Recovery helps teens build motivation, cope with urges, manage thoughts and emotions, and develop healthier behaviors that support lasting change. When clinically indicated, teens may also receive additional recovery coaching sessions to provide more individualized support throughout treatment.

Frequently Asked Questions

What is the average length of stay for PHP?

The average length of stay for PHP in our Elk Grove mental health program is 2–4 weeks.

Our Elk Grove teen PHP runs Monday through Friday from 9am to 4pm. A typical day includes individual therapy, group therapy, DBT skills training, a daily clinical check-in, academic time, family therapy, and experiential therapy (art and/or music). Weekly psychiatry is also built into the schedule.

If you’re considering PHP for your teen, here are some of the ways it can help:

  • Structured Treatment: PHP provides a consistent daily routine with therapy sessions and activities, helping teens build healthy habits and a stronger sense of stability.
  • Clinical Support: Teens have regular access to therapists, psychiatrists, and counselors, so they feel supported and guided throughout treatment.
  • Integration with Daily Life: Unlike inpatient care, teens return home at the end of the day, giving them the opportunity to apply what they are learning in real-life situations and build confidence over time.
  • Community and Peer Support: Group therapy sessions allow teens to connect with others facing similar challenges, creating a sense of support and shared accountability.
  • Personalized Treatment Plans: Evolve’s PHP programs are tailored to each teen, with flexibility to address co-occurring conditions, substance use, and individual goals.

Suicidal Thoughts, Self-Harm, or Substance Use:

If a teen is experiencing suicidal thoughts, engaging in self-harm that does not require medical attention, or struggling to stay sober. Teens should generally be able to keep themselves safe without intensive monitoring during evenings and weekends.

Disrupted Daily Life:

If a teen is having trouble maintaining relationships or is struggling to function at home, with peers, or in their community. This may include difficulty keeping up with daily responsibilities, refusing to attend school, or a noticeable decline in academic performance related to mental health, substance use, or behavioral challenges.

Teens in Evolve’s PHP meet with their assigned psychiatrist on a weekly basis. Evolve’s board-certified child and adolescent psychiatrists are full-time members of the care team and stay closely involved in each teen’s treatment. They regularly collaborate with therapists and the Clinical Director to ensure care stays aligned and responsive to each teen’s needs.

At the start of treatment, the psychiatrist connects with parents or caregivers to introduce themselves and learn more about the teen’s history. If any medication changes are recommended, the psychiatrist or nurse will reach out to walk through the details, answer questions, and obtain consent.

Medication is always optional, and no teen is required to take it as part of their care.

At Evolve, teens in PHP stay engaged with school through dedicated academic time each weekday, with about two hours set aside Monday through Friday. Parents and therapists work closely with school counselors to coordinate assignments or weekly coursework, helping teens stay on track while in treatment.

Laptops are available for research or online classes as needed, and staff are present during academic time to provide support, keep teens focused, and encourage productive habits. The goal is to help teens maintain progress in school while building the skills they need to manage responsibilities more independently.

The average length of stay for IOP is 2-3 months.

Our Elk Grove teen IOP runs Monday through Thursday from 4pm to 7pm. A typical week includes group therapy, DBT skills training, individual therapy, family therapy, multi-family group, and experiential therapy (art or music), with bi-weekly or monthly psychiatry sessions. The after-school schedule is designed to let teens continue school and daily routines while still getting structured clinical support.

An IOP offers a middle ground between outpatient care and 24/7 residential treatment. It’s designed for teens who need structured support for mental health or substance use, while still being able to stay engaged in their day-to-day lives. Here are some of the top benefits of an IOP for teens:

  • Flexibility: IOP allows teens to receive treatment while continuing with school, family life, and other daily responsibilities.
  • Comprehensive Care: Treatment includes a mix of group therapy, individual sessions, and targeted approaches that support both mental health and substance use needs.
  • Supportive Environment: Teens have the opportunity to connect with peers facing similar challenges, creating a sense of support and shared accountability.
  • Tailored Treatment: Each plan is personalized, with care adjusted to meet the teen’s specific needs, challenges, and goals.

Suicidal Thoughts, Self-Harm, or Substance Use:

If a teen is experiencing suicidal thoughts, engaging in self-harm that does not require medical attention, or having difficulty maintaining sobriety. Teens should generally be able to stay safe without intensive monitoring during evenings and weekends.

Disrupted Daily Life:

If a teen is struggling to maintain relationships or having difficulty functioning at home, with peers, or in their community. This may show up as trouble keeping up with daily responsibilities or managing expectations across different areas of life.

Teens in Evolve’s IOP meet with their assigned psychiatrist on a monthly basis. Evolve’s board-certified child and adolescent psychiatrists are full-time members of the care team and stay actively involved in each teen’s treatment. They regularly collaborate with therapists and the Clinical Director to ensure care remains consistent and aligned with each teen’s needs. Medication is always optional, and no teen is required to take it as part of their care.

We were very fortunate to have my daughter (16) in Evolve. She was there for a few weeks, but her stay helped her stabilize and find some serenity. I noticed the improvement soon, and we, the parents, felt relief as well.
