Evolve San Jose Outpatient is located in the Rose Garden neighborhood, just off Highway 87 and I-880, making it an accessible option for families attending PHP or IOP several days a week. We serve families throughout the South Bay, including San Jose, Santa Clara, Campbell, and Willow Glen. Our location is close to Abraham Lincoln High School and other San Jose Unified School District schools, helping teens stay connected to their local school communities while in treatment. How We Approach Substance Use Treatment

For teens struggling with substance use or addiction, Evolve incorporates SMART Recovery, a science-based approach rooted in CBT. SMART Recovery helps teens build motivation, cope with urges, manage thoughts and emotions, and develop healthier behaviors that support lasting change. When clinically indicated, teens may also receive additional recovery coaching sessions to provide more individualized support throughout treatment.

What is the average length of stay for PHP?

The average length of stay for PHP in our San Jose mental health program is 2–4 weeks.

Our San Jose teen PHP runs Monday through Friday from 9am to 4pm. A typical day includes individual therapy, group therapy, DBT skills training, a daily clinical check-in, academic time, family therapy, and experiential therapy (art and/or music). Weekly psychiatry and medication management are also built into the schedule.

If a teen is experiencing suicidal thoughts, engaging in self-harm that does not require medical attention, or having difficulty maintaining sobriety. Teens should generally be able to remain safe without the need for intensive monitoring during evenings and weekends.

Disrupted Daily Life:

If a teen is struggling to maintain relationships or function at home, with peers, or in their community. This may include difficulty keeping up with daily responsibilities, refusing to attend school, or a noticeable decline in academic performance related to mental health, substance use, or behavioral challenges.

Teens in Evolve’s PHP meet with their assigned psychiatrist on a weekly basis. Evolve’s board-certified child and adolescent psychiatrists are fully integrated into the care team and remain closely involved in each teen’s treatment, collaborating regularly with therapists and the Clinical Director to keep care aligned with evolving needs.

At the start of treatment, the psychiatrist connects with parents or caregivers to introduce themselves and better understand the teen’s history. If any medication changes are recommended, the psychiatrist or nurse will follow up to review details, answer questions, and obtain consent.

Medication is always optional and is never required as part of care.

At Evolve, teens in PHP stay engaged with school through dedicated academic time each weekday, with about two hours set aside Monday through Friday. Parents and therapists coordinate with school counselors to align assignments and coursework, helping teens stay on track during treatment.

Laptops are available for research or online classes as needed, and staff are present to provide support, maintain focus, and reinforce productive habits. The goal is to help teens keep up with school while building skills to manage responsibilities more independently.

Our San Jose teen IOP runs Monday through Thursday from 4pm to 7pm. A typical week includes group therapy, DBT skills training, individual therapy, family therapy, multi-family group, and experiential therapy (art or music), with regular psychiatry check-ins as clinically indicated. The after-school schedule is designed to let teens continue school and daily routines while still getting structured clinical support.

An IOP provides a level of care between traditional outpatient services and full-time residential treatment. It’s designed for teens who need structured support for mental health or substance use, while still staying involved in their day-to-day lives. Here are some of the key benefits of an IOP for teens:

  • Flexibility: IOP allows teens to receive treatment while continuing with school, family life, and other daily responsibilities.
  • Comprehensive Care: Programs include a combination of group therapy, individual sessions, and targeted approaches that address both mental health and substance use.

Teens in Evolve’s IOP meet with their assigned psychiatrist on a monthly basis. Evolve’s board-certified child and adolescent psychiatrists are full-time members of the care team and remain actively involved in each teen’s treatment, working closely with therapists and the Clinical Director to keep care aligned and consistent. Medication is always optional and is never required as part of treatment.
