Evolve Elk Grove offers two outpatient program levels of care for teens ages 12-17 who are experiencing mental health, substance use, and/or behavioral challenges. Partial Hospitalization Program (PHP)

Evolve’s Partial Hospitalization Program (PHP), also known as a “full day program”, offers teens a comprehensive day of treatment five days a week.

If a teen is experiencing suicidal thoughts, engaging in self-harm that does not require medical attention, or struggling to stay sober. Teens should generally be able to keep themselves safe without intensive monitoring during evenings and weekends.

Disrupted Daily Life:

If a teen is having trouble maintaining relationships or is struggling to function at home, with peers, or in their community. This may include difficulty keeping up with daily responsibilities, refusing to attend school, or a noticeable decline in academic performance related to mental health, substance use, or behavioral challenges.

Teens in Evolve’s PHP meet with their assigned psychiatrist on a weekly basis.

Evolve’s board-certified child and adolescent psychiatrists are full-time members of the care team and stay closely involved in each teen’s treatment. They regularly collaborate with therapists and the Clinical Director to ensure care stays aligned and responsive to each teen’s needs.

At the start of treatment, the psychiatrist connects with parents or caregivers to introduce themselves and learn more about the teen’s history. If any medication changes are recommended, the psychiatrist or nurse will reach out to walk through the details, answer questions, and obtain consent.

Medication is always optional, and no teen is required to take it as part of their care.

At Evolve, teens in PHP stay engaged with school through dedicated academic time each weekday, with about two hours set aside Monday through Friday. Parents and therapists work closely with school counselors to coordinate assignments or weekly coursework, helping teens stay on track while in treatment.

Laptops are available for research or online classes as needed, and staff are present during academic time to provide support, keep teens focused, and encourage productive habits. The goal is to help teens maintain progress in school while building the skills they need to manage responsibilities more independently.

Yes, at PHP, teens will complete random Urine Drug Screens weekly. 

Intensive Outpatient Program (IOP)

At Evolve’s Intensive Outpatient Program (IOP), also called an “after-school program”, teens receive a half day of programming several days a week.

An IOP offers a middle ground between outpatient care and 24/7 residential treatment. It’s designed for teens who need structured support for mental health or substance use, while still being able to stay engaged in their day-to-day lives. Here are some of the top benefits of an IOP for teens:

  • Flexibility: IOP allows teens to receive treatment while continuing with school, family life, and other daily responsibilities.
  • Comprehensive Care: Treatment includes a mix of group therapy, individual sessions, and targeted approaches that support both mental health and substance use needs.

Family involvement is a key part of care at Evolve. In our PHP and IOP programs, teens and their parents or caregivers participate in weekly family therapy sessions focused on building stronger communication and practical skills. Our therapists use a solution-focused, skills-based approach, helping families learn and practice strategies they can carry into everyday life. Parents also receive guidance and education to better understand what their teen is experiencing and how to support them outside of treatment.
